Are all pages of the cookbook numbered?

Do you index cookbooks for the author? If not, once I have finished all of my entries, can you send me the electonically laid out book so that I can index it and add the index to the back of the book?

If neither is possible, what solutions have other cookbook authors used to make finding recipes easy?
